In this study, 383 patients with hepatitis B virus-related cirrhotic portal hypertension were followed up for 11 years to investigate the impact of laparoscopic splenectomy and azygoportal disconnection (LSD) on hepatocellular carcinoma (HCC) risk. The LSD group demonstrated higher survival rates and lower incidence of HCC compared to the endoscopic therapy group. Logistic regression analysis confirmed that LSD was an independent protective factor against HCC. These findings suggest that LSD can improve postoperative survival and prevent HCC in patients, making it a valuable alternative to liver transplantation in the context of donor shortages.
