In a nationwide study comparing robotic and open pancreatoduodenectomy, 701 patients underwent the robotic procedure while 4447 had the open one with similar major complications, mortality rates, and pancreatic fistula outcomes. Robotic pancreatectomy had longer operating times but resulted in less blood loss, fewer infections, and shorter hospital stays, particularly in centers performing over 20 robotic surgeries annually which had lower mortality and conversion rates. Randomized trials are warranted to confirm the benefits of robotic over open pancreatoduodenectomy.
