External validation of the Scoring System of Appendicitis Severity (SAS) fell short in distinguishing complicated appendicitis. The SAS 2.0 was developed and externally validated, showing high accuracy in predicting complicated appendicitis in patients. It provides individual probability estimates with a 95% confidence interval, aiding in decision-making for nonoperative treatment.
