Researchers examined the concept of total neoadjuvant therapy (TNT) for pancreatic ductal adenocarcinoma (PDAC), finding that the term “total” is not consistently defined. Although it has gained popularity for its promise of comprehensive preoperative treatment, the current understanding and protocols vary widely, with inconsistent therapy durations and approaches. Among 7 studies reviewed, the length of TNT varied from 2 to 4 months, often including postoperative adjuvant therapy, which contradicts the TNT concept. The findings suggest a lack of clear evidence to support TNT’s use in pancreatic cancer.
