Nonoperative management of acute complicated diverticulitis with pericolic and/or distant extraluminal air is feasible and safe, with an 88% success rate among patients. While pericolic extraluminal air had a 93.1% success rate, distant extraluminal air had a 71.1% success rate. Distant subgroup showed higher failure rates but remains a challengingly feasible treatment option, with 18.3% distant recurrence rate and 11.3% elective surgery rate.
