Changes in J-Pouch Phenotypes Over Time

Endoscopic pouch phenotypes in patients with inflammatory bowel disease can transition over time, with normalization of pouches associated with favorable outcomes. Subsequent development of diffuse inflammation, pouch-related fistulas, and afferent limb/inlet stenoses significantly worsen pouch outcomes. Patients with pouch inflammation who achieve subsequent normalization have lower pouch excision risks. Understanding these changes is crucial for managing patients post-proctocolectomy with ileal pouch-anal anastomosis.
