A retrospective analysis of 25,848 pancreaticoduodenectomy patients revealed that 10.5% had preoperative hypoalbuminemia, which significantly correlated with increased 30-day complications. Specifically, hypoalbuminemic patients faced higher risks for serious complications (OR 1.80) independent of other comorbidities but did not have increased mortality risk (OR 1.37). The median age of hypoalbuminemic patients was notably higher, and many were classified as ASA class 4 or higher, emphasizing the need for preoperative optimization based on albumin levels.
