In patients with stage IIA rectal cancer above the peritoneal reflection who did not receive preoperative chemoradiotherapy, adjuvant chemotherapy (AC) demonstrated significantly improved 5-year overall survival (93.2% vs. 84.6%, p = .001) and recurrence-free survival (81.6% vs. 64.1%, p = .01). Patients with one or more risk factors showed marked benefits from AC compared to those who did not receive it. However, no survival differences were observed in patients older than 65 years.
