In a study comparing laparoscopic distal gastrectomy (LDG) and open distal gastrectomy (ODG) in older adults with locally advanced gastric cancer, LDG showed significantly lower rates of postoperative infectious complications. Among matched patients, only 3.4% of those undergoing LDG experienced complications compared to 27.6% for ODG. Though LDG had a longer operative time, it also resulted in lower estimated blood loss and fewer overall postoperative complications, indicating it as a safer option for patients aged ≥75 years.
