Preoperative recurrence score predicts early recurrence in cholangiocarcinoma

A preoperative recurrence score (PRS) model was developed to predict early recurrence in peri-hilar cholangiocarcinoma patients based on radiological features. In a study of 215 patients, significant predictors included tumor size, portal vein involvement, hepatic artery involvement, and suspicious lymph nodes. The PRS demonstrated excellent discrimination, achieving AUCs of 0.83 in internal and 0.84 in external validation cohorts. A high PRS correlated with a substantial risk of recurrence, suggesting reconsideration of upfront surgery for high-risk patients.
