In patients with inflammatory bowel disease (IBD) and primary sclerosing cholangitis (PSC), subtotal colectomy may be a viable surgical option for those with rectal sparing disease, as it was found to have no dysplasia or neoplasia in the residual colon or rectum. Endoscopic surveillance showed that 2% of patients who underwent total proctocolectomy with ileal pouch-anal anastomosis (IPAA) developed low-grade dysplasia in the ileal pouch or rectal cuff, while 1% developed neoplasia in the rectal cuff after long-term follow-up.
