Utilizing three-dimensional reconstruction, clinicians performed a successful laparoscopic left hepatectomy and caudate lobectomy in a patient with left perihilar cholangiocarcinoma. The reconstruction revealed tumor invasion into the left hepatic artery and left portal vein. The procedure lasted 425 minutes with minimal blood loss. The patient’s postoperative recovery was smooth, and no recurrence was observed at the 6-month follow-up. This study suggests that 3D reconstruction-guided laparoscopic radical resection can enhance precision and efficiency in selected perihilar cholangiocarcinoma cases, potentially yielding outcomes comparable to open surgery.
