Tumor size correlates with biochemical activity in pheochromocytomas, with lesions ≥2.3 cm more likely to produce levels 3 times upper limit of normal. Doubling tumor size or a ≥1 cm increase enhances biochemical activity. While paroxysmal symptoms are common, they don’t significantly correlate with tumor size or biochemical activity. These findings suggest tumor size can guide the timing of operative intervention for pheochromocytoma.
