Simultaneous cholecystectomy during elective colorectal cancer surgery for patients with asymptomatic gallstones reduces long-term biliary complications without increasing postoperative morbidity. Female gender, old age (≥65 years old), and small multiple gallstones are identified as independent risk factors for developing gallstone-related diseases in patients who undergo elective colorectal surgery without simultaneous cholecystectomy. Prophylactic cholecystectomy is recommended for older patients, females, and those with small multiple calculi.
