Time kinetics of calcitonin normalization (CN) and biochemical persistence/recurrence (BP) after surgery for medullary thyroid carcinoma were studied in 190 patients. Early CN predicted long-term risks of recurrence, with lymph node ratio (LNR) being a significant factor. Higher primary tumor size and longer CN time were associated with decreased disease-free survival. Tailored follow-up based on these findings is crucial for improving patient outcomes.
