Comparative analysis revealed that single anastomosis sleeve ileal (SASI) bypass leads to greater weight loss than sleeve gastrectomy (SG) with a mean difference of 11.32 kg, and improves metabolic parameters, including type 2 diabetes mellitus (RR=1.35), dyslipidemia (RR=1.41), and obstructive sleep apnea (RR=1.50). No significant differences were found compared to one-anastomosis gastric bypass (OAGB), and complication rates were similar across all groups. Further research with extended follow-up is recommended.
