Evidence from a multi-institutional cohort study indicates that adjuvant therapy significantly improves overall survival rates in patients with high-risk periampullary adenocarcinomas. Among 736 patients analyzed, those receiving adjuvant therapy had a median overall survival of 68.7 months compared to 61.1 months for those under observation, translating to a hazard ratio of 0.73. Additionally, nodal involvement further worsened survival outcomes, underscoring the critical importance of adjuvant therapy in these cases.
