A novel nomogram, based on liver stiffness (ls) via 2D-sheared wave elastography and spleen area (spa), was developed to predict posthepatectomy liver failure (PHLF) in hepatocellular carcinoma patients. Analysis of 562 patients demonstrated superior diagnostic performance compared to conventional models, such as the MELD and ALBI scores. In the training cohort, the nomogram achieved an area under the curve (AUC) of 0.833, while in the validation cohort, it showed an AUC of 0.802, indicating high predictive accuracy.
