Elderly patients experience worse outcomes in acute appendicitis.

Patients aged 65 and older with acute appendicitis presented later, requiring more diagnostic imaging and experiencing higher rates of complications compared to younger adults. The study revealed that elderly individuals had longer symptom durations (7.88 days vs. 3.56 days) and required CT scans more frequently (86.1% vs. 54.0%). Surgical delays were notable, with fewer elderly patients operated on within 24 hours (85% vs. 88.7%). Ultimately, elderly patients faced significantly increased postoperative complications and longer hospital stays.
