A systematic review and meta-analysis involving 930 patients revealed that anatomic resection (AR) significantly enhances both overall survival (OS) and disease-free survival (DFS) compared to non-anatomic resection (non-AR) for resectable intrahepatic cholangiocarcinoma. The OS rates at 1, 3, and 5 years were 71.5%, 46.1%, and 34.3% for AR versus 63.6%, 32.9%, and 24.8% for non-AR. DFS also favored AR, indicating its oncologic superiority without impacting postoperative outcomes.
