An observational study across twelve high-level trauma centers identified 178 failures in the re-triage process for severely injured patients. Key critical failures included insufficiently trained transportation staff, ineffective transmission of imaging, and incomplete clinical information exchange. The average re-triage time was noted at four hours, while a two-hour timeframe could reduce mortality risks. Findings suggest a need for enhanced training and communication protocols to mitigate these failures and improve patient outcomes.
