A multicenter cohort study analyzed data from 1,479 patients with clinical stage II/III low rectal cancer in Japan, identifying key risk factors for local recurrence (LR). The 5-year cumulative incidence of LR was found to be 12.3%. Notable preoperative, treatment-related, and postoperative factors were linked to increased LR risk. Of particular concern, patients without neoadjuvant therapy demonstrated significantly higher LR risk when presenting with two or three preoperative risk factors, including elevated BMI.
