In laparoscopic enhanced totally extra-peritoneal inguinal hernia repairs, balloon trocar dissection outperforms optical trocar entry by significantly shortening surgery duration (62 vs. 75 min, p<0.001) and accelerating preperitoneal space creation (13 vs. 19 min, p<0.0001). It also minimizes peritoneal breaches (0% vs. 10.34%, p<0.002) and postoperative complications, including lower pain scores and reduced seroma and hematoma rates (1.72% vs. 17.24%, p<0.004). This technique simplifies the learning curve for surgeons.
