Left-sided pancreatectomy with splenectomy shows no advantage in reducing in-hospital mortality or major complications compared to nonsplenectomy procedures in traumatic pancreatic injuries.
- 758 patients analyzed: 51.8% underwent splenectomy; no significant difference in mortality (adjusted odds ratio 0.81) or major adverse events (adjusted odds ratio 0.97) between groups.
- Splenectomy patients had shorter time to operation (64 vs 519 minutes).
Surgeons should consider this data in individualized decision-making for splenectomy during left-sided pancreatectomy, keeping in mind that long-term complications still need further research.
