Combined and staged surgery had similar complication rates and survival in a French national cohort.
- Severe complications occurred in 39.7% of combined and 45.3% of staged patients (p = 0.20).
- Median overall survival was 52.4 months with combined surgery and 57.3 months with staged surgery (p = 0.30).
- Major hepatectomy independently raised severe complication risk (adjusted OR 1.81).
Neither strategy was clearly safer or more effective, so sequencing should be decided case by case by the multidisciplinary team, with major hepatectomy as a recognized risk factor for complications.
