Mapping the surgical discharge process in US hospitals revealed key components and challenges specific to surgical patients. Current care transition models do not address wound care education, pain control, non-home post-discharge locations, and follow-up plans. Challenges include social situations, team communication issues, and post-discharge care coordination. Addressing these components and challenges in care transitions for surgical patients can lead to improved outcomes and reduced care fragmentation.
