Most gastric leiomyomas are benign subepithelial tumors, with few becoming symptomatic or increasing in size. A retrospective review of 231 cases showed that only 2.4% experienced size increase, one of which was leiomyosarcoma. The majority remained stable, with resections confirming leiomyoma. Close monitoring may be sufficient for asymptomatic cases, but resection could be considered for those showing ulceration or size increase.
