Middle Colic Artery: A Key Landmark in Laparoscopic Pancreatoduodenectomy

Exposing the middle colic artery (MCA) first in laparoscopic pancreatoduodenectomy allows for safe and efficient determination of the superior mesenteric artery (SMA), first jejunal artery (J1A), and inferior pancreatoduodenal artery (IPDA). An anatomical correlation study found that 90.9% of MCA started at 12-3 o’clock from SMA, with mean distances of 56.4 mm to MCA, 37.4 mm to J1A, and 19 mm between SMA and J1A. This technique facilitates complete dissection of the mesopancreas and lymph nodes.
