Impact of Major Hepatectomy on Recurrence in HCC Patients

Patients with hepatocellular carcinoma undergoing major hepatectomy had higher complication rates and shorter recurrence-free survival compared to those undergoing minor hepatectomy, regardless of propensity score matching. Major hepatectomy was identified as an independent predictor for HCC recurrence. Minor hepatectomy resulted in fewer postoperative complications and better recurrence-free survival outcomes. Therefore, parenchyma-sparing resection should be favored for HCC at CNLC ιb stage.
