Minor resections may be preferred for large hepatocellular carcinoma

A multicenter study analyzed 535 patients with single large hepatocellular carcinoma (slhcc). Results indicated that major hepatectomy led to significantly higher major complications and no long-term survival benefits compared to minor resections, especially left lateral sectionectomy, which was associated with better overall survival (p=0.02). The absence of oncological benefits from major resection suggests minor resections should be favored when feasible and oncologically appropriate, promoting improved surgical outcomes for slhcc patients.
