Surgery following neoadjuvant chemotherapy significantly improves overall survival for patients with borderline resectable and locally advanced unresectable pancreatic ductal adenocarcinoma, according to a retrospective study involving 124 patients. After applying methods to reduce selection and immortal time biases, analysis revealed marked survival advantages for those undergoing surgery compared to chemotherapy alone. This finding was supported through propensity score matching and Cox regression analyses across various cohorts, reinforcing the benefits of surgical intervention in this patient population.
