Perioperative DNR Status Linked to Higher Early Mortality

Continuing a non-full code status during surgery is associated with increased early postoperative mortality, indicating a need to reassess code status decisions in this vulnerable population.

  • Among 2,833 patients, 3-day mortality was 2.9% for those who remained not full code, compared to 1.2% for those who transitioned to full code (adjusted odds ratio 2.18).
  • The absolute risk difference underscores a potential 1.43% increase in mortality for non-full code patients.

Surgeons should carefully evaluate the appropriateness of existing DNR orders before procedures, as they may impact postoperative outcomes significantly.
