Deciphering heterogeneous tertiary lymphoid structures (TLS) patterns in colorectal cancer revealed three clusters with different prognostic implications. High metabolic activity and stromal pathways linked to worse outcomes, while immune activation pathways indicated improved prognosis and better response to immunotherapy. A new TLScore based on 9 core genes showed potential for guiding treatment decisions, highlighting the promise of personalized medicine in colorectal cancer.
