Deep Learning Improves Prediction of Response to Neoadjuvant Chemotherapy in Colorectal Liver Metastasis

Deep learning models applied to prechemotherapy imaging accurately predict response to neoadjuvant chemotherapy in colorectal cancer liver metastases, outperforming clinical models. A study on 95 patients found the deep learning model had an AUC of 0.77, superior to the clinical model’s AUC of 0.41, showing potential to identify nonresponders and guide patient management towards curative resection.

Criteria for Early Drain Removal after Hepatectomy

Identifying predictors of bile leakage after hepatectomy, researchers established criteria for safe early drain removal. Analyzing 351 patients, they found a drainage fluid direct bilirubin level cutoff of 0.16 mg/dl and volume cutoff of 125 ml on postoperative day 2. Patients meeting both criteria had no leakage. These findings offer a novel approach for timely drain removal, potentially reducing complications and improving postoperative management.

Effectiveness of Ultrasound-Guided Ablation Techniques for Benign Thyroid Nodules

Ultrasound-guided ablation techniques, particularly radiofrequency ablation (RFA), show effectiveness in treating benign thyroid nodules. RFA with 2 sessions is optimal for solid nodules, while polidocanol ablation stands out for cystic nodules. A network meta-analysis of 35 studies involving 5655 patients demonstrated RFA as the most effective method at 12 months, with RFA2 showing strong outcomes at 6 months. Hospital Volume Impacts Rectal Cancer Surgery Outcomes

Higher hospital surgical volume correlates with significantly better short- and long-term outcomes after rectal cancer surgery. Patients treated in hospitals with higher surgical volume experienced lower 90-day and 1-year excess postoperative mortality rates and higher 5-year observed survival rates compared to those treated in lower-volume hospitals.

Outcome of Laparoscopic Peritoneal Lavage for Perforated Diverticulitis

Laparoscopic lavage for non-feculent perforated diverticulitis shows lower mortality, shorter hospital stays, and fewer subsequent surgeries, compared to sigmoid resection. It is a safe alternative for select patients, especially those younger, healthier, and with less previous abdominal surgeries. Laparoscopic lavage offers promising outcomes as a treatment option.

Factors Influencing Long-Term Survival After Pancreatic Ductal Adenocarcinoma Resection

Impact of Triclosan-Coated Sutures on Surgical Site Infections in Abdominal Surgery

Triclosan-coated sutures significantly reduce the incidence of surgical site infections (SSI) after abdominal surgery by 2%, as demonstrated in a meta-analysis of 11 randomized controlled trials with over 10,000 patients. This study found lower SSI rates in the triclosan-coated group compared to uncoated sutures, particularly with polydioxanone and polyglactin 910 sutures.
