SKA3 – A Potential Therapeutic Target in Cancer Precision Surgery

Researchers explored the role of spindle and kinetochore-associated complex subunit 3 (SKA3) in cancer. SKA3, a key component in chromosomal segregation and cell division, has been identified as having oncogenic properties contributing to tumor development. This review integrates bibliometric analysis, pan-cancer studies, and narrative summary to assess SKA3’s role in cancer. The authors also identify the top 10 predicted drugs that target SKA3. This work suggests that targeting SKA3 could provide new therapeutic opportunities for precision surgery in cancer treatment.

Organizational Factors of New Pancreas Units: Insights from Surgical Leaders

Pancreas units, a new model of care, rely on multidisciplinary meetings, team leadership, and performance metrics as key organizational factors. Surgical leaders stress the importance of involving clinical professionals and patients in decision-making. Non-technical skills like ethics and teamwork are valued, with mentoring and training as crucial facilitating factors. Trust in functional leaders, key performance indicators, and support roles like nurse navigators is strong. Pancreas units offer potential for improved patient outcomes and professional benefits for surgeons.

Early Recurrence Prediction in Hepatocellular Carcinoma Patients

Pre-operative enhanced MRI combined with clinical features can predict early recurrence of hepatocellular carcinoma after radical resection. Patient age, microvascular invasion, tumor boundary, and large vessel invasion were identified as independent predictive factors. The combined predictors had an area under the curve of 0.861, suggesting a reasonable predictive model for early recurrence of HCC.

Management of Nontraumatic Spontaneous Renal Hemorrhage through Robotic-Assisted Laparoscopic Nephrectomy

Researchers described three cases of Wünderlich Syndrome (nontraumatic spontaneous renal hemorrhage) treated with robot-assisted laparoscopic nephrectomy. The patients, aged 44 to 53, presented with flank pain and were diagnosed with Wünderlich Syndrome via CT scan. Each patient underwent a retroperitoneal robotic-assisted nephrectomy after conservative treatment failed. Pathological examination revealed angiomyolipoma in one case and renal clear cell carcinoma in the other two. At the 9-month follow-up, patients exhibited good renal function with no evidence of recurrence or metastasis. This case series demonstrates the potential of robotic-assisted laparoscopic nephrectomy in effectively managing Wünderlich Syndrome.

Acupuncture Point Stimulation Improves Tolerance in Non-Sedated Upper GI Endoscopy

Researchers conducted a double-blinded, randomized controlled trial to examine the effects of transcutaneous electrical nerve stimulation of acupuncture points (acu-tens) during non-sedated oesophagogastroduodenoscopy (OGDS). The trial included 348 subjects undergoing elective diagnostic OGDS. The success rate of OGDS was significantly higher with acu-tens (77.8%) than with placebo (68.0%; OR 1.64, 95% CI 1.01-2.66, p = 0.043). Acu-tens also increased patients’ willingness for future OGDS and shortened procedure duration. No adverse effects were reported, suggesting acu-tens could be a safe adjunct for non-sedated OGDS.

Comparing Gastrointestinal Stromal Tumor Outcomes Between Geriatric and Non-Geriatric Patients

Population-based analysis shows lower overall 5-year survival rates in geriatric gastrointestinal stromal tumor patients compared to non-geriatric patients. Surgery significantly improves survival rates in both groups, with geriatric patients having higher hazard death ratio. Clinical management for geriatric patients may benefit from large prospective studies.

Standardized Key Procedural Steps for Endoscopic Sleeve Gastroplasty

Experts established 21 key steps for safe and effective endoscopic sleeve gastroplasty (ESG) through a modified Delphi method. Consensus was reached on the importance of full-thickness bites and the use of a helix device in tissue swirling, although opinions varied on minor details like fundus inclusion. This standardized protocol will enhance ESG training and adoption by endoscopists.

Effectiveness and Safety of Endoscopic Resection for Gastric Gastrointestinal Stromal Tumors

Researchers investigated the efficacy and safety of endoscopic resection (ER) for gastric gastrointestinal stromal tumors (GISTs) originating from the muscularis propria (MP). The study included 233 patients who underwent ER, with a median tumor size of 12 mm. The complete resection rate was 93.1%, with complications occurring in 4.7% of cases, including perioperative perforations (1.7%) and bleeding (1.3%). Factors associated with piecemeal resection were larger tumor size and irregular shape. The findings suggest ER is an effective and reasonably safe approach for gastric GISTs originating from the MP.

Remote Programming for Sacral Neuromodulation: Feasibility and Patient Satisfaction

Researchers conducted a multicenter prospective study to assess remote programming’s effectiveness and patient satisfaction in stage I sacral neuromodulation (SNM). A total of 63 participants from 6 centers were divided into remote programming (RP) and outpatient control (OC) groups. The RP group showed high usability (98%) and satisfaction (96.83%). It significantly improved patient scores on the ICSI/ICPI scale compared to the OC group (median difference -13.50 vs -2, p = 0.015). There were no significant differences in phase-two conversion rates or quality of life. This study suggests that remote programming is a viable alternative to in-person programming for stage I SNM patients.

Sleeve Gastrectomy Shows Promising Effects on Metabolic Dysfunction-Associated Fatty Liver Disease (MAFLD)

Researchers investigated the diagnostic accuracy of transient elastography (TE) for steatosis and liver fibrosis in patients with metabolic dysfunction-associated fatty liver disease (MAFLD) undergoing sleeve gastrectomy (SG). TE’s controlled attenuation parameter (CAP) and liver stiffness measurement (LSM) showed high accuracy in identifying liver steatosis and fibrosis. A nomogram was developed based on multivariate logistic regression analysis to predict severe MAFLD. After a 6-month follow-up, SG led to significant reductions in steatosis grade, fibrosis stage, and metabolic indicators in patients with obesity and MAFLD.
