Unplanned conversion during laparoscopic liver resection for hepatocellular carcinoma is associated with higher blood loss, transfusion rate, hospital stay, and postoperative complications. However, the 5-year overall survival rate was similar in patients who underwent unplanned conversion and those who underwent laparoscopic liver resection.

Sizing protocol (pop+2 vs pop+3) in magnetic sphincter augmentation did not impact outcomes like dysphagia, ph normalization, or need for dilation or removal. Upsizing did benefit patients with poor peristalsis or hypercontractile esophagus. Device size increase correlated with demographic and preoperative characteristics, suggesting it is a surrogate for esophageal circumference.

Recent studies have confirmed the long-term safety and efficacy of laparoscopic distal gastrectomy compared to open gastrectomy for locally advanced gastric cancer. Ongoing trials are evaluating perioperative immunotherapy for resectable gastric cancers. Key trials like dante, gercor neonipiga, keynote-859, and spotlight are exploring targeted therapies for unresectable or metastatic gastroesophageal cancers, incorporating immune checkpoint inhibitors and the monoclonal antibody zolbetuximab targeting cldn18.2.

A clinical model was developed to predict postsurgical intra-abdominal infection in blunt abdominal trauma patients based on a retrospective analysis of 553 patients from 7 medical centers. Independent risk factors were identified, including age, prehospital time, c-reactive protein, injury severity score, operation duration, intestinal injury, neutrophils, and antibiotic use. The nomogram showed good predictive performance in both derivation and validation cohorts, indicating potential for clinical application in guiding treatment decisions.

Restorative proctocolectomy with ileal pouch-anal anastomosis for ulcerative colitis improves quality of life but may not fully resolve extraintestinal disorders like primary sclerosing cholangitis, joint, skin, and eye issues. Treatment of pouchitis does not impact primary sclerosing cholangitis, but biologic therapies for pouch inflammatory disorders may benefit joint, skin, and eye manifestations.

Simulation of 20,000 obese patients with non-alcoholic steatohepatitis found that sleeve gastrectomy (SG) led to a gain of 14.3 years compared to medical weight management (MWM), with only 5% of SG patients requiring liver transplantation (LT) after one year, compared to 9% in the MWM group. SG reduced the need for LT in patients with a BMI above 32.2 kg/m, highlighting the potential of bariatric surgery in managing obesity and non-alcoholic steatohepatitis.

A single institution retrospective analysis of rectal cancer cases before and after the adoption of a nonoperative “watch and wait” pathway found no significant increase in abdominoperineal resection for patients without a complete clinical response to neoadjuvant therapy. Although there was a higher rate of pathologic complete responses in the post-“watch and wait” period, restaging some patients with clinical near-complete responses may be necessary to balance outcomes.

Patients with gallbladder cancer presenting with acute cholecystitis had a 20% overall 5-year survival rate, with stage-specific survival rates varying. A third of patients with gallbladder cancer presented with acute cholecystitis but did not experience significantly different survival outcomes compared to other presentations. Age, gender, and the presence of bile spillage during cholecystectomy did not significantly impact survival. These findings suggest that outcomes in patients with gallbladder cancer and acute cholecystitis are comparable to those found incidentally.
