Scissor-Type Knives Show Similar Efficacy in Colorectal Endoscopic Submucosal Dissection

Scissor-type knives, clutch cutter (CC) and SB knife Jr2 (SB), demonstrated comparable efficacy in colorectal endoscopic submucosal dissection. Both groups exhibited high en bloc resection rates and low complication rates, with CC showing significantly less perioperative bleeding than SB. Risk factors for prolonged procedure time included antiplatelet use, larger lesion size, severe fibrosis, and higher perioperative bleeding frequency. These findings highlight the feasibility of using scissor-type knives for colorectal ESD procedures.

Neoadjuvant Chemoradiotherapy Yields 84% 3-Year Survival for T4/N2 Rectal Cancer

Neoadjuvant chemoradiotherapy followed by total mesorectal excision resulted in a 3-year overall survival rate of 84% for patients with T4 and/or N2 rectal adenocarcinoma. Achieving R0 resection improved recurrence and survival rates, with T4 disease showing worse outcomes than N2. Adjuvant chemotherapy was associated with a survival benefit in the whole cohort and the N2 subgroup, but not in the T4 subgroup. Personalizing treatment strategies based on high-risk features could enhance outcomes in locally advanced rectal cancer.

Immune and Oxidative Stress-Related Genes as Targets for mRNA Vaccines in Pancreatic Cancer

Identification of potential mRNA vaccine targets in pancreatic cancer patients using immune- and oxidative stress-related genes. A model incorporating ERAP2, MET, CXCL9, and AGT genes predicts patient prognosis more accurately than current models. ERAP2 aids immune activation and cancer immune evasion, MET regulates cancer cell proliferation and invasion, CXCL9 expression is linked to poor prognosis in pancreatic cancer, and AGT cleavage impacts patient survival. These genes could serve as promising targets for mRNA vaccine development in pancreatic cancer, warranting further investigation.

3D Liver Reconstruction Enhances Surgical Planning and Accuracy

A systematic review of 47 studies involving 3D liver reconstruction highlights its benefits in liver surgery. 87.2% of studies used 3D reconstructions for preoperative planning, particularly for complex hepatectomies and liver grafts. The approach improved navigation and accuracy in 55.3% of cases, facilitated graft selection and vascular preservation in living donor liver transplants, and allowed accurate measurement of the future liver remnant in 19.1% of studies. Overall, 3D liver reconstruction proves valuable for enhancing surgical precision and planning in liver surgery.

New Classification System Improves Laparoscopic Treatment of Extrahepatic Choledochal Cyst

Oncology Nurse Navigator Cuts Delays and Boosts Patient Satisfaction

A dedicated Oncology Nurse Navigator significantly reduced diagnostic and treatment delays, resulting in quicker access to care for cancer patients. Patients in the study group experienced shorter wait times for diagnostic testing, multi-disciplinary team discussions, oncology visits, and surgery scheduling compared to the control group. The introduction of the Oncology Nurse Navigator led to increased patient satisfaction, with improved follow-up outcomes and decreased post-discharge issues. This study highlights the importance and effectiveness of implementing a specialized nurse navigator role in improving the quality and efficiency of surgical oncology patient care pathways.

Augmenter of Liver Regeneration Impacts Blunt Liver Trauma

The study explores the role of Augmenter of Liver Regeneration (ALR) in blunt liver trauma using a rat model. ALR levels in plasma and liver tissue showed significant fluctuations depending on the time post-trauma. Plasma ALR levels peaked at 30 minutes, while intact liver tissue ALR levels were highest at 15 and 30 minutes. Inflammation was greater as time progressed, with significant differences observed in ALR levels across various time points. This is the first study to analyze ALR’s role in isolated blunt liver trauma and its potential impact on liver repair.

Novel scoring system accurately predicts risk of failure to achieve textbook outcomes after hepatectomy for hepatocellular carcinoma

A multicenter study developed and validated a scoring system to predict the risk of not achieving textbook outcomes (non-TOS) following hepatectomy for hepatocellular carcinoma (HCC). The scoring system, based on factors like obesity, ASA score, Child-Pugh grade, tumor size, and extent of hepatectomy, accurately stratified patients into low, intermediate, and high-risk categories for non-TOS. The novel system identified 39.6% of patients who failed to experience a textbook outcome, with the predicted and observed risks closely matching in the validation cohort. This score may aid in early risk identification and surgical decision-making for HCC patients.

Novel Biomarkers for Predicting Acute Mesenteric Ischaemia

A prospective multicentre study aims to identify biomarker combinations that can accurately diagnose acute mesenteric ischaemia (AMI) and distinguish between reversible and irreversible bowel ischaemia. Serum biomarkers including I-FABP, Alpha-GST, IL-6, PCT, IMA, D-lactate, D-dimer, SCUBE-1, and LBP will be measured in adult patients with suspected AMI. The study will develop a practical score for diagnosing AMI and guiding treatment decisions based on the identified biomarker combinations, potentially offering an improved method for early diagnosis and treatment planning in patients with AMI.
