Identification of pulmonary ventilation function abnormalities, age over 70, BMI over 27 kg/m2, and porcine small intestine submucosal mesh as independent risk factors for incisional hernia recurrence after open preperitoneal repair. Significant differences in recurrence and non-recurrence patients noted in factors such as mesh materials, surgical site infections, and hernia defect width. These findings provide critical insights for long-term management strategies.

Radiological Marker Correlates with Tumor Budding in Pancreatic Cancer

Pancreatic cancer’s radiomorphological appearance on MRI correlates with tumor budding, a marker of poor prognosis. Intermediate to high spiculation on MRI was seen in 64% of tumors, with over 90% showing tumor buds. A positive correlation existed between MRI spiculation grade and histopathological tumor bud density. This radiological marker may help identify patients needing extensive surgery or personalized therapies, offering a non-invasive estimation of tumor behavior.

Artificial intelligence predicts non-transplantable recurrence in hepatocellular carcinoma

Artificial intelligence models accurately predicted non-transplantable recurrence in hepatocellular carcinoma patients post-hepatic resection. An ensemble AI model showed high accuracy (AUC 0.751-0.858) with incorporation of postoperative factors. Radiologic tumor burden and microvascular invasion were key predictors. Patients with predicted recurrence had lower survival rates, highlighting the models’ prognostic value. The AI models could guide treatment decisions and personalize postoperative care for better outcomes.

Learning Curve Effects of TransREctus Sheath PrePeritoneal Procedure on Inguinal Hernia Repair

The Transrectus Sheath PrePeritoneal procedure for inguinal hernia repair showed safety and efficacy, with a low rate of complications and hernia recurrences. Analysis of 422 procedures revealed no significant differences in outcomes based on surgeon experience level, suggesting a short learning curve. Implementation of the procedure did not result in a notable increase in adverse events. This study highlights the feasibility and effectiveness of the Trepp technique in high-volume surgical practice.

Prognostic Model for Unresectable Gastric Cancer Liver Metastases

Study establishes a reliable nomogram model with five independent risk factors for gastric cancer liver metastases. The model accurately predicts patient prognosis, showing promising predictive accuracy and clinical benefit for outcome evaluation. Results demonstrate high area under the curve values at 1, 3, and 5 years, excellent consistency in calibration curves, and substantial net clinical benefit in decision curve analysis.

Reduced postoperative sore throat with combined budesonide and dexamethasone

Preoperative inhaled budesonide combined with intravenous dexamethasone significantly decreased postoperative sore throat (post) incidence and severity at 6, 12, and 24 hours in patients undergoing thyroidectomy. Additionally, this combination resulted in lower postoperative coughing rates at 24 hours compared to intravenous dexamethasone alone. Hoarseness outcomes were similar between the two groups. This combination therapy shows promise in reducing postoperative discomfort in these patients.

Overlap Esophagojejunostomy With Self-Pulling and Latter Transection Technique in Laparoscopic Total Gastrectomy

An innovative technique called overlap self-pulling and latter transection was evaluated in laparoscopic total gastrectomy for esophagojejunostomy. Results showed similar postoperative outcomes compared to the standard technique, but with an increased duration and cost. This method is considered safe and feasible for anastomosis in gastric cancer patients undergoing laparoscopic total gastrectomy.

Management and Prognosis of Ruptured Hepatocellular Carcinoma

Spontaneous rupture of hepatocellular carcinoma (RHCC) presents a life-threatening complication in 3-15% of patients with HCC. Recent studies show improved survival benefits with transcatheter arterial embolization followed by elective hepatectomy. Despite poor prognosis, active surveillance and multidisciplinary management strategies can enhance short- and long-term outcomes. Achieving hemostasis and stabilizing patients remain crucial in RHCC management, with mortality rates ranging from 25-75% in earlier studies.

Indocyanine Green Fluorescence for Lymph Node Metastasis in Gastric Cancer

Indocyanine Green (ICG) fluorescence in laparoscopic subtotal gastrectomy for gastric cancer showed a sensitivity of 75.86% in detecting metastatic lymph nodes and 90.7% in identifying them. ICG-guided lymphadenectomy revealed higher fluorescent lymph node numbers in patients with metastases. The method offers high negative predictive value and clear visualization of the lymphatic system, indicating effectiveness in radical gastrectomy for gastric cancer.

Factors associated with loss to follow-up in surgical trials

Loss to follow-up (LTFU) is uncommon in surgical randomized controlled trials. Characteristics associated with reduced LTFU include standard-of-care outcome assessments, surgery for transplantation or cancer. Increased LTFU risk is seen in trials with patient-reported outcomes and longer follow-up periods. Understanding these factors can help investigators anticipate and address LTFU to ensure trial integrity.
